On the early morning hours of July 23, 1982, actor Vic Morrow, and two juvenile Asian actors Renee Chen and My-ca Dinh Le were killed during an accident on set.

While filming a Vietnam-set battle sequence where a hovering helicopter is chasing and shooting at Morrow, whom is carrying two Vietnamese children out of a deserted village and across a small lake, a SFX explosion caused the helicopter to crash on top of them, killing all three instantly.

Renee Chen and My-ca Dinh Le were not officially part of the cast, and their parents had been paid in cash because it is illegal for children to work at that hour (the crash was at about 2:30 am).

In 1987, director John Landis and four others crew members were tried and found not guilty of involuntary manslaughter.
